USA Hockey verifies player participant ages by reviewing the players government issued Birth Certificate once during the player's membership with USA Hockey. Using the secure portal, a players documents are reviewed by the Utah Registrar. A player becomes eligible to participate with a USAH team when they have completed the verification of their date of birth, have properly registered and completed required certifications and applicable requirements.

Managing Disputes

Disagreement vs Policy Violation vs SafeSport Violation

Disagreements involving differences of opinions, dissatisfaction with a player's game time, line placement, practice drills, for example, should be brought to one's team manager for resolution. These are not policy violations.

Allegations of policy violations involving breaches of USA Hockey or Utah Amateur Hockey Association (UAHA) policies and/or rules should be addressed first at the team or jr Mustangs administration level when possible and appropriate. If no resolution, the complaint can be filed with UAHA following their procedures outlined in the "Dispute Resolution Procedure" document provided herein.

SafeSport violations are those of a Sexual, Physical or Emotional Abuse; Harassment, Sexual Harassment, Bullying, Threats, Hazing. To report actual or perceived abuse, utilize the proper USA Hockey Safesport procedures for reporting. A Reporting form and link is provided herein. Educational materials are provided here. Incidents of a criminal nature should be immediately reported to law enforcement authorities and also to USA Hockey's Safesport portal.
