Which Travel Consent Form do I Use?
Annual Consent for Dual Relationship. A parent may authorize a non-parent Adult to transport, lodge, have one-to-one interactions with an adult, one-to-one interactions with an adult in Locker Room (injury treatment), Electronic Communications with an adult (i.e. Instruction), or other situation described by parent.
Annual Consent to authorize Individual Training Sessions, Athletic Training, Transportation by an Adult, for a Minor Athlete; Organization Transportation by Bus; Media and Championship Celebrations in the Locker Room.
